196例结核病住院患者抑郁和焦虑情绪的现状及其影响因素分析

    目的 探讨结核病住院患者抑郁与焦虑情绪的流行现状及影响因素,为临床实施心理社 会支持的综合干预措施提供依据。方法 采用横断面研究设计,2026 年1 月20 日—2 月15 日采用便利 抽样法选取在首都医科大学附属北京胸科医院结核科住院患者247 例为研究对象。采用一般资料调 查表收集患者社会人口学和临床特征资料,使用患者健康问卷抑郁自评量表(PHQ-9)和广泛性焦虑障 碍量表(GAD-7)分别评估患者抑郁情绪与焦虑情绪的严重程度。采用多因素Logistic 回归分析结核病 住院患者抑郁和焦虑情绪的影响因素。结果 共发放问卷247 份,获得有效问卷196 份,有效回收率为 79.4%。196 例结核病住院患者抑郁情绪(PHQ-9 总分≥ 5 分)检出率为48.5%(95/196),焦虑情绪(GAD-7 总分≥ 5 分)检出率为31.6%(62/196),抑郁与焦虑共病(PHQ-9 总分≥ 5 分,且GAD-7 总分≥ 5 分)率为 28.6%(56/196)。单因素分析结果显示,不同家庭人数、慢性病、年龄、体重指数(BMI)、贝达喹啉使用情 况的结核病住院患者抑郁发生率比较,差异有统计学意义(χ2/Z=8.557、2.421、-2.014、-2.967、7.463; 均P< 0.2)。不同家庭人数、耐药情况、贝达喹啉使用情况和年龄结核病住院患者的焦虑发生率比较, 差异有统计学意义(χ2/Z=3.908、3.839、1.807、-2.419;均P< 0.2)。校正混杂因素后,结核病住院患 者抑郁和焦虑情绪影响因素的多因素Logistic 回归分析显示,BMI 高(OR=0.905,95%CI:0.831~0.984, P=0.020)、家庭人数多(OR=0.140,95%CI:0.037~0.529,P=0.004)是结核病住院患者发生抑郁的独立保 护因素;年龄大(OR=0.976,95%CI:0.957~0.995,P=0.013)、家庭人数多(OR=0.299,95%CI:0.101~0.889, P=0.030)是结核病住院患者发生焦虑的独立保护因素。结论 结核病住院患者抑郁、焦虑及情绪共病 检出率较高,心理问题较为普遍。BMI高、家庭人数多是患者抑郁情绪的独立保护因素,年龄大、家庭 人数多是其焦虑情绪的独立保护因素。临床应重点关注低BMI、低龄、家庭人数较少的结核住院患者, 开展针对性心理干预,有效缓解患者负性情绪,促进病情恢复。

    Abstract:

    Objective To investigate the prevalence and influencing factors of depressive disorder and anxiety in hospitalized patients with tuberculosis, so as to provide a basis for implementing comprehensive psychosocial support interventions in clinical practice. Methods Using the cross-sectional study design, 247 hospitalized patients from the Department of Tuberculosis of Beijing Chest Hospital, affiliated with Capital Medical University, were conveniently selected as study subjects between January 20 and February 15, 2026. Patients' socio-demographic and clinical baseline information were collected using the General Information Questionnaire, and the severity of depression and anxiety was assessed using the Patient Health Questionnaire-9( PHQ-9) and Generalized Anxiety Disorder-7( GAD-7), respectively. Multivariate Logistic regression analysis was used to explore the influencing factors of depressive disorder and anxiety in hospitalized patients. Results A total of 247 questionnaires were distributed, with 196 valid responses, resulting in a valid response rate of 79.4%. The prevalence of depressive disorder( PHQ-9 total score ≥5) among 196 hospitalized tuberculosis patients was 48.5%( 95/196), the prevalence of anxiety( GAD-7 total score≥5) was 31.6%( 62/196), and the prevalence of comorbid depressive disorder and anxiety( PHQ-9 total score ≥5 and GAD-7 total score ≥5) was 28.6%( 56/196). Univariate analysis showed that there were significant differences in the prevalence of depressive disorder among hospitalized tuberculosis patients with different family size, chronic disease, age, body mass index( BMI) and use of Bedaquiline( χ2/Z=8.557, 2.421,-2.014, -2.967,7.463; all P < 0.2). There were statistically significant differences in the prevalence of anxiety among hospitalized tuberculosis patients with different family size, drug resistance, use of Bedaquiline and age( χ2/Z=3.908, 3.839, 1.807, -2.419; all P< 0.2). After adjusting for confounding factors, multivariate Logistic regression analysis showed that high BMI[ OR=0.905, 95%CI( 0.831, 0.984), P=0.020] and large family size[ OR=0.140, 95%CI( 0.037, 0.529), P=0.004] were independent protective factors for depressive disorder in hospitalized patients with tuberculosis. Advanced age[ OR=0.976, 95%CI( 0.957, 0.995), P=0.013] and larger family size[ OR=0.299, 95%CI( 0.101, 0.889), P=0.030] were independent protective factors for anxiety in hospitalized patients with tuberculosis. Conclusions Hospitalized tuberculosis patients have a high incidence of depressive disorder, anxiety, and emotional comorbidity, and psychological issues are relatively common. High BMI and large family size are independent protective factors of depressive disorder in patients, while advanced age and family size are independent protective factors of anxiety. Clinical attention should be focused on hospitalized tuberculosis patients with low BMI, young age and small family size, and targeted psychological intervention should be carried out to effectively alleviate the negative emotions of patients and promote the recovery of the disease.
